Instructions and forms – Group of two to five persons

Read the following instructions carefully. You must fill out three forms.


The undertaking covers the principal sponsored person and, if applicable, his or her spouse and dependent children. A separate undertaking agreement must be signed for any other person who does not fit the definition of a spouse, de facto spouse or dependent child (e.g., parents of the principal sponsored person, child who does not meet the definition of “dependent child”).

On the sponsorship application, it is essential to declare:

- All members of the immediate family of the person you are sponsoring (spouse and dependent children), even if they are not accompanying the sponsored person - Members of the sponsored person’s immediate family (spouse and dependent children) who are missing if you do not have confirmation that they are deceased


1. Undertaking application form

Be sure to complete all sections of the Formulaire d’engagement – Parrainage collectif – Groupe de deux à cinq personnes form A-0546-CF (In french only PDF, 629 ko) (Undertaking Application Form – Collective Sponsorship – Group of Two to Five Persons), paying special attention to the following sections:

Section 1, box A - You must designate someone as the group’s spokesperson. This person must be among the sponsors. When this person obtains information on the file, he or she must inform the other group members.

Section 2, box B - “Accompanying family members of the principal sponsored person” – You must enter the names of the persons who fit the definition of family member and are living with the principal sponsored person or whose contact information you have and who will be able to arrive in Québec with the principal sponsored person..

Section 2, box C - “Non-accompanying family members of the principal sponsored person” – You must enter the names of any family members who would accompany the principal sponsored person if they could. For example, this could be a spouse or dependent child whose whereabouts are unknown or who is in his or her country of origin or some other country and whose application cannot be processed simultaneously by Immigration, Refugees and Citizenship Canada. It is important to include this person on the undertaking form and inform the principal sponsored person that he or she must also include this person on his or her application for permanent residence.

Section 4 In the reception and integration plan, you must identify the person(s) who will provide each of the services offered to the sponsored persons.

Section 8 – Sign the form in the space provided, and enter the date and location.

2. Sponsor’s Individual Record

Each member of your group must fill out and sign the Fiche individuelle du garant – Personne physique A-0538-F (In French only - PDF, 69 Ko)

  • To protect the confidentiality of the information you provide, insert this form and the supporting documents requested in a sealed envelope and attach it to the Undertaking Application form.
  • Please complete all sections that apply to your situation. An incomplete form will be returned to you, which will delay the processing of your application. Be sure to sign Section 11.
  • In sections 1.3 and 1.6, enter the last and first names of your spouse and dependent children, if applicable. In Section 2, you must also provide us with complete information concerning undertakings that you or your spouse have already initiated.
  • See the list of offences before checking off the appropriate box for the fifth declaration in Section 3.

3. Sponsor’s or Co-signatory Spouse’s Declaration of Authorization

If you have been separated or divorced, or if you have children from a previous union, you must fill out and sign the Autorisation pour la cueillette et la communication de renseignements personnels A-0527-FO (In French only - PDF, 67 ko). Your spouse does not have to fill out this form, even if his or her income is factored in when the family income is calculated.

Carefully read the provisions for verifying information. Please be advised that if you refuse to allow the information to be verified, your application will be denied.

Other documents required from sponsors

Documents required from each group member

A copy of the document attesting to:

  • Canadian citizenship (citizenship card or birth certificate) or
  • Permanent residence (Record of Landing or Permanent Resident Card)

A copy of the last personal notice of assessment issued by Agence du Revenu du Québec

A copy of all your latest tax slips showing your income: Relevé 1 or other slips


If you are an employee:

  • The original letter from each of your current employers. This letter should specify the number of hours worked per week, the date you started work and, if applicable, the expected end date of employment, your gross annual salary and the cumulative gross earnings for the current calendar year
  • For each of your current jobs, a copy of your last two pay slips
  • A copy of all employment records issued during the last 12 months for employment insurance purposes indicating termination of employment or layoff, if applicable

Additional documents to provide if your working income is insufficient

  • A copy of any official and nominative documents showing additional income such as pensions, permanent government benefits, etc.
  • A copy of your latest account statement (investments, RRSPs, or TFSAs) or the original of a letter from a financial institution stating the opening date and balance of your accounts, term deposits, RRSPs, etc.
  • A copy of proof of bond ownership and the related interest rate
  • If you own a property, a copy of your most recent municipal property tax bill with the standard value of your property on it
  • A document attesting to the balance of your mortgage:
    • The original letter from your financial institution stating your mortgage balance or
    • A copy of a recent statement of your mortgage account showing the address of the property and your mortgage balance or
    • A certificate issued by the Land Register of Québec indicating that the property is mortgage-free

Please note that current accounts and chequing accounts will not be considered.

Documents required from sponsored persons, to be submitted with the sponsorship application

A photocopy of the passport or other identification document of each sponsored person



Ministère de l’Immigration, de la Francisation et de l’Intégration may verify or have verified with third parties the accuracy of the information provided on your application. It may reject an undertaking application because it contains false or misleading information or documents and may refuse to consider an undertaking application from a person who has provided false or misleading information or documents for two years or less. The Ministère may also initiate criminal proceedings against the sponsor if it provides false or misleading information.

You must return all required forms and documents in an envelope with sufficient postage. The documents will not be returned to you. It is therefore advisable to send copies except when originals are required.
