Instructions, forms and documents to include – Group of two to five persons

Read the following instructions carefully. You must fill out three forms.

The undertaking covers the principal sponsored person and, if applicable, his or her spouse and dependent children. A separate undertaking agreement must be signed for any other person who does not fit the definition of a spousede facto spouse or dependent child (e.g., parents of the principal sponsored person, child who does not meet the definition of “dependent child”).

On the sponsorship application, it is essential to declare:
- All members of the immediate family of the person you are sponsoring (spouse and dependent children), even if they are not accompanying the sponsored person
- Members of the sponsored person’s immediate family (spouse and dependent children) who are missing if you do not have confirmation that they are deceased.

Documents to provide to the MIFI when submitting an undertaking application

Please note that if your group is represented by an immigration consultant or a mandated lawyer, all documents submitted to the MIFI must be written in French. If you submit translated supporting documents, you must submit the original translation completed by a recognized translator.

All documents submitted in a language other than French will be returned to the legal representative and considered missing.

1. Undertaking application form

If you meet the requirements and want to submit a collective sponsorship application, here is the procedure to follow:

Be sure to complete all sections of the Formulaire d’engagement – Parrainage collectif – Groupe de deux à cinq personnes form A-0546-CF (In French only PDF, 629 Kb) (Undertaking Application Form – Collective Sponsorship – Group of Two to Five Persons), paying special attention to the following sections:

Section 1, box A - You must designate someone as the group’s spokesperson. This person must be among the sponsors. When this person obtains information on the file, he or she must inform the other group members.

Section 2, box B - “Accompanying family members of the principal sponsored person” – You must enter the names of the persons who fit the definition of family member and are living with the principal sponsored person or whose contact information you have and who will be able to arrive in Québec with the principal sponsored person..

Section 2, box C - “Non-accompanying family members of the principal sponsored person” – You must enter the names of any family members who would accompany the principal sponsored person if they could. For example, this could be a spouse or dependent child whose whereabouts are unknown or who is in his or her country of origin or some other country and whose application cannot be processed simultaneously by Immigration, Refugees and Citizenship Canada. It is important to include this person on the undertaking form and inform the principal sponsored person that he or she must also include this person on his or her application for permanent residence.

Section 4 In the reception and integration plan, you must identify the person(s) who will provide each of the services offered to the sponsored persons.

Section 8 – Sign the form in the space provided, and enter the date and location.

2. Documents required from each sponsor

2.1 Sponsor's Individual Record

Each member of your group must fill out and sign the Fiche individuelle du garant – Personne physique A-0538-F (In French only - PDF, 69 kB).

To protect the confidentiality of the information you provide, insert this form and the supporting documents requested in a sealed envelope and attach it to the Undertaking Application form.

Read the “General Information” page carefully in order to complete all the sections that apply to your situation. An incomplete form will be returned to you, which will delay the processing of your application.

Please fill out:

Section 1 – All information requested is mandatory

Sections 1.3 to 1.6 - indicate the name(s) of your spouse and, if applicable, dependent children, writing “N/A” in cases that do not apply to your situation.

Section 2 – Provide us with complete information concerning undertakings that you or your spouse have already initiated.

Section 3 – All information requested is mandatory. Consult the list of offences from the Department of Justice Canada.

The Ministère may reject any applications that contain false or misleading information or documents (see Section 57 of the Québec Immigration Act).

Sections 4, 5 and 6 – Information about your income. Reply to all the questions that apply to your situation and provide all supporting documents that correspond to the sections filled out.

Section 7, 8 and 9 – Information on your spouse’s income. Your spouse, who is a member of your group of sponsors, can fill out this section or fill out a separate, individual file.

Section 10 – Read carefully.

Section 11 – Read the declaration carefully and sign the form.

Read the provisions concerning verification of the information carefully. Please be advised that refusal to authorize the information to be verified will result in rejection of your application.

Section 12 – Read the consent carefully and sign the form.

2.2 A copy of your document attesting to:

  • Canadian citizenship (citizenship card or birth certificate) or
  • permanent residence (Record of landing or Permanent Resident Card)


2.3 Sponsor's or Spouse's Declaration of Authorization

If you have been separated or divorced, or if you have children from a previous union, you must fill out and sign the Autorisation pour la cueillette et la communication de renseignements personnels A-0527-FO (In French only - PDF, 67 kB.

Read the provisions for verifying information carefully. Please be advised that if you refuse to allow the information to be verified, your application will be denied.

Documents required from sponsored persons, to be submitted with the sponsorship application

A photocopy of the passport or other identification document of each sponsored person.


The Ministère de l’Immigration, de la Francisation et de l’Intégration may verify or have verified with third parties the accuracy of the information provided on your application. It may reject an undertaking application because it contains false or misleading information or documents and may refuse to consider an undertaking application from a person who has provided false or misleading information or documents for two years or less. The Ministère may also initiate criminal proceedings against the sponsor if he or she provides false or misleading information.


You must return all required forms and documents in an envelope with sufficient postage. The documents will not be returned to you. It is therefore advisable to send copies except when originals are required.
