List of documents to be included - Joint undertaking formula

To submit an application, please provide the following documents:

• The Undertaking – Collective Sponsorship – Joint Undertaking Application – Québec Resident and Legal Person form (Dynamic PDF, 181 Kb) duly completed and signed
• A copy of the corporate charter or the incorporating document
• A copy of a Resolution of the board of directors of an organization interested in signing an undertaking (Dynamic PDF, 32 Kb)
• Proof of the Québec resident’s citizenship or permanent residency and, if applicable, proof of his or her financial capacity
• The Sponsor Individual Record – Collective Sponsorship – Group and Joint Application Undertaking (Dynamic PDF, 163 Kb), completed and signed by the Québec resident
• The Application for Permanent Residence in Canada form from Citizenship and Immigration Canada (strongly recommended)

Please note that forms must be filled out in French. The English translations (specimen) are included only to help you do so accurately.

If you are not an organization that has signed an umbrella agreement, you must send us:

• Proof of your financial capacity, such as balance sheet, income and expense statement, annual budget, and other similar documentary evidence
• A copy of financial statements from your last fiscal year, accompanied by a review engagement report signed by an accountant (CA, CGA or CMA)

	USEFUL INFORMATION
	The Ministère may verify directly or by means of a third party the accuracy of the information provided in your application. The Ministère may reject an application that contains false or misleading information or documents and may refuse to consider an undertaking application from someone who has provided false or misleading information or documents within the past two years. The Ministère may also take legal proceedings against a sponsor if he or she provides false or misleading information.
