For any application to be submitted to one of our offices in Québec
If you wish to pay by credit card, please attach this form duly completed
to your application.
For any application to be submitted to one of our offices outside Québec
Consult the pages of the
Québec immigration office covering your country for the methods of payment
available. To access these pages, select the country
where you legally reside:
Downloading, printing and saving forms
- To view or print PDF files, you must download the Adobe
Reader program available for free on the Adobe website.
- You can complete on line the forms marked "dynamic." Print,
sign and forward them by mail.
- It is possible to save the dynamic PDF form using Acrobat Standard (version
6.0 or higher) and a minimal version of Windows XP. To save, click on the "Save" button
on the Acrobat Reader toolbar.
